Why Your Neck Hurts Worse When You Wake Up

Why Your Neck Hurts Worse When You Wake Up

Waking up with a stiff neck once in a while is annoying — waking up with a stiff neck every day is downright debilitating. Persistent morning neck pain is one of the most common complaints we get from patients. The first step to finding relief? Understanding why it’s happening in the first place.

That’s where Dr. David Wu and our team at CurePain come in.

We’re here to explain the “how come” behind your morning neck pain

It’s your sleeping position

Sleeping at a funky angle puts strain on your neck and can leave you waking up in pain day after day. When your head is tilted too far forward, backward, or to the side for hours at a time, it can stress the muscles and joints of the cervical spine.

Stomach sleepers are at the highest risk, as it forces your neck to stay rotated for long periods. Side and back sleeping are usually better options, as long as you have proper support.

Speaking of proper support…

It’s your pillow

Your pillow plays a major role in how your neck feels in the morning. If it’s too high or too flat, you fall asleep with your neck in an unnatural position — that misalignment leads to muscle tension, stiffness, and even headaches when you wake up.

Ideally, we want your pillow to keep your head in a neutral position. Memory foam or cervical pillows are especially helpful as they cradle your head and mold to its contours, providing consistent support throughout the night.

Or your mattress

Pillows take most of the heat for being less-than-supportive, but your mattress could be just as much of a problem. A mattress that’s too soft or too firm can throw your entire body out of alignment, putting extra pressure on your neck.

If your mattress is old or no longer supportive, it may be contributing to your pain without you realizing it.

It’s your stress levels

Stress isn’t just a burden on your mind — it can also affect your body. If you’re living with chronic stress, you may be unconsciously tensing your neck and shoulder muscles during the day and night.

As the tension builds, so does your daily pain. In some cases, tension in the neck and shoulder muscles can trigger bruxism (teeth grinding or clenching), which can exacerbate neck pain.

It’s an old injury

If you’ve had a previous neck injury, such as whiplash or a sports-related strain, you may be more prone to morning discomfort. Conditions such as arthritis, herniated discs, or degenerative changes in the spine can also cause stiffness that worsens after a night of inactivity.

When the body is still for several hours, joints can stiffen, and inflammation can settle in, making movement more uncomfortable when you first wake up.

How to reduce morning neck pain

Making a few small adjustments can go a long way in improving how your neck feels in the morning. We usually have you start by switching to a more supportive pillow, evaluating your mattress, and/or adjusting your sleep position.

Gentle stretching in the morning can also help loosen tight muscles and improve mobility. Applying heat or taking a warm shower may further relieve stiffness and promote circulation.

If your neck pain stems from a more serious condition, such as spinal stenosis or cervical radiculopathy, we can discuss more advanced treatments, including radiofrequency therapy (RFA), physical therapy, and corticosteroid injections.

Wake up feeling better

Morning neck pain isn’t something you have to live with. By addressing the underlying causes, you can take steps toward more restful nights and more comfortable mornings.
